Review of colposcopy activities in a level 2 hospital in Dakar

Abstract

Objectives: To take stock of colposcopy activities and describe the management of lesions objectified by colposcopy at the Military Hospital of Ouakam.


Patients and methods: It was a prospective and retrospective study conducted over a period of one year (12 months) and concerning patients who underwent colposcopy at the Military Hospital of Ouakam. We studied the following parameters: the socio-demographic characteristics of the patients, the results of the colposcopy and the histological examination and the treatment. Data were analyzed using Epi info version 7 software.


Results: Fifty-seven patients met our inclusion criteria. Their epidemiological profile was a woman with an average age of 40, married (79%), multiparous (54.4%), in a period of genital activity (73.3%). The examination after application of acetic acid often showed a normal appearance (73.7%), mild acidophilia (12.3%) or homogeneous acidophilic areas (8.8%). After application of lugol, the most frequent results were a normal appearance (64.9%) or an iodo-negative zone (29.4%). At the end of the examination, we had recorded 32 normal colposcopies (56.1%), 8 atypical grade 1 transformations (14%), 8 atypical grade 2 transformations (14%). Treatment consisted of vaginal hysterectomy in 11 cases (19.3%). Histological examination of the surgical specimen found a normal cervix (27.3%), CIN1 (36.4%), CIN2 (27.3%) or CIN3 (9.1%).


Conclusion: This work shows that colposcopy makes it possible to specify the diagnosis of smears considered indeterminate. However, its place must be refocused in the HPVtest / cytology-colposcopy-biopsy tripod.
