Maternal-fetal prognosis of planned cesarean section compared to emergency cesarean section: realities in rural areas.

Abstract

Objective : evaluate the indications for cesarean section in the two groups while verifying the benefit of the programming with regard to maternal and fetal prognosis.  Methodology : This is a retrospective descriptive and analytical case control study ranging from the period between January 1 to December 2020, at the maternity of Tivaouane. The study concerned all women who gave birth by cesarean section during this period. The following were included : cases (all patients who had an emergency section) and controls (all patients who had a planned cesarean section). The data were collected through delivery record with file maker Pro software and data analysis with R studio software version 1.2.1335. Results : During the study period on a total of  2,810 deliveries we recorded 1757 low-lying deliveries, i.e. 62.5% and 1053 caesarean sections or 37.5%.Among the 1053 caesarean, we performed 866 emergency caesarean sections or 82.2% and 187 prophylactic caesarean sections or 17.8%. The average age was

The proportion of patients with a history of caesarean section was higher in the control group 49.2% versus 7.9% (p< 0,0001). Regarding caesarean indications according to Robson's classification, the proportion of patients in group three (3) and group one (1) is the largest in the case group 25.3% to 21.1% than in the control group  (p< 0,0001). We recorded 7% perinatal deaths among patients who had an emergency caesarean section compared to 2.1% in patients who had a scheduled caesarean section (p=0,019). There were five maternal deaths, all of which were related to emergency caesarean sections  Conclusion: Overall, prophylactic caesarean section is a surgical procedure that improves the maternal prognosis, which is perinatal unlike emergency caesarean section.
