Abstract
Vulvar hematoma or vulvar thrombus is most often seen in the immediate postpartum period after vaginal delivery. Outside of labor, this situation is rare. We report a case of a massive vulvar hematoma in a 20-year-old nulliparous woman with a pregnancy of 19 weeks after a violent intercourse. On examination, the patient was agitated, hemodynamically stable, with a painful, shiny swelling measuring 17 cm × 12 cm in diameter, located on the left large and small lip. She had undergone surgical treatment with incision of the nympho-hymenal fold, drainage of the hematoma followed by a hemostatic suture with 2/0 Vicryl. The postoperative course was simple and she was discharged on day 2. Healing was obtained on the 10th day and the pregnancy progressed well. She gave birth at term by vaginal route to a newborn female, weighing 2700 g, and the score Apgar 8-9 after five minutes, after a
sutured episiotomy. The postpartum period was simple.
